Makati Medical Center Outpatient Requirements
As part of its preventive measures against the spread of the Coronavirus Disease 2019 (COVID-19), Makati Medical Center (MMC) requires all its patients, including their companions, to submit an online screening form prior to visiting MMC.
The form can be accessed through
this link, or by visiting MMC’s website at
www.makatimed.net.ph.
COVID-19 Screening Process:
- Go to www.makatimed.net.ph then click the Online Screening Form button
- Agree with the Data Privacy Consent
- Fill out the required fields then click Submit
- Wait for the confirmation email
- Present the confirmation email prior to your visit
This form is valid for 5 days. Companions are also required to fill out the COVID-19 Screening Form.
Patients and their companion must comply with the standard prevention and protection protocols inside MMC:
- No Mask, No Entry
- Thermal Scanning
- 1 Companion Policy
- Physical Distancing Protocol
- Limited Seating Capacity, at least 2 meters apart
