Follow these steps to enroll existing dependents:

You must confirm their enrollment to ensure continued coverage. 

Step 1: Log in to www.benefitsmadebetter.com. 

Step 2: Click “Dependent’s Enrollment” tab. 

Step 3: Click “Proceed to Dependents Enrollment” to start the enrollment process. 

Step 4: Review your dependent’s information. Once done, click “Enroll Dependent/s”. 

Step 5: Click “DECF and Data Privacy Consent Forms.” 

Step 6: Read the Dependents Enrollment Confirmation Form and select “Download DECF and Data Privacy Consent”. 

To accept, you must enter the confirmation code that will be sent to your registered mobile number or email address. 

 

  • If you exceed 3 attempts at SMS confirmation code delivery, the code will be emailed to you. 
  • If you do not confirm using either SMS or email, you will receive a text-to-speech call with the code. 
  • If you do not receive a code, please wait for 5-15 minutes before trying again.  Confirm your acceptance through Two-Factor Authentication (2FA). 

 

Remember that completing Two-Factor Authentication (2FA) automatically confirms your dependents’ endorsement to the HMO plan. Your dependents’ enrollment cannot be cancelled until the next renewal cycle unless they meet removal criteria. 

 

Do you have dependents aged 18 and above? Submit their signed Data Privacy Consent Form. 

 

Existing dependents don’t need to submit any other supporting documents. 

 

Updating, removing, or adding new dependents? 

Click “Request for Dependents Reset.” ActiveLink will validate your request and notify you within 48 hours. Once your dependents are reset, you may enroll using the steps for new dependents.
