Follow these steps to enroll existing dependents:
Step 1: Log in at
www.benefitsmadebetter.com.
Step 2: Click “Dependent’s Enrollment” tab.
- Click “Proceed to Dependents Enrollment” to start the enrollment process (Step 3).
- Click “Forfeit” if you do not wish to enroll dependents. You will be taken to the Dependents Enrollment window.
If you change your mind, you can click “Re-Open” to unlock the module and enroll dependents. Note that you can only do this once and only during the 30-day enrollment period.
Step 3: Review your dependent’s information. Once done, click “Enroll Dependent/s.”
Step 4: Click “DECF and Data Privacy Consent Forms.”
Read the Dependents Enrollment Confirmation Form. Confirm your acceptance through Two-Factor Authentication (2FA).
- To accept, you must enter the confirmation code that will be sent to your registered mobile number or email address.
- If you exceed 3 attempts at SMS confirmation code delivery, the code will be emailed to you.
- If you do not confirm using either SMS or email, you will receive a text-to-speech call with the code.
- If you do not receive a code, please wait for 5-15 minutes before trying again.
Remember that completing Two-Factor Authentication (2FA) automatically confirms your dependents’ endorsement to the HMO plan. Your dependents’ enrollment cannot be cancelled until the end of the policy year unless they meet removal criteria.
- Do you have dependents aged 18 and above? Submit their signed Data Privacy Consent Form. You don’t need to submit any other supporting documents.
To update, remove or add new dependent/s, click
“Request for Dependents Reset.” ActiveLink will validate your request and notify you within 48 hours. Once your dependents are reset, you may enroll using the steps for new dependents.
